Question

I am 24 weeks pregnant and a 4D ultrasound showed the fetus has hydrops in the left kidney, with the left kidney measuring 3921mm and the right kidney 3116mm. The anterior-posterior diameter of the separated dark area in the left renal collecting system is about 12mm, and the width of the dark area on the right side is about 5mm. Both sides have a renal cortex thickness of about 6mm, and the bladder measures 181523mm. After consulting with a genetic prenatal consultation, I was told that a termination of pregnancy might be necessary, but both my husband and I are in good health, and I don’t know what to do now.

Answer

Hello, please do not worry too much. I suggest you observe for another two weeks. According to the current ultrasound report, the anterior-posterior diameter of the separated dark area in the left renal collecting system is indeed beyond the normal range at about 12mm, but the width of the dark area on the right side is still within a reasonable range, indicating that bilateral ureteral atresia can be ruled out. Additionally, excessive urination by the fetus may also lead to such results. I recommend you have another check-up in two weeks, preferably around 3 pm when the fetus’s intrauterine metabolism is mostly completed for a more accurate result. Wishing you and your baby good health.
