Question

What are the treatment and management strategies for hepatitis syndrome?

Answer

The primary goal of treating hepatitis syndrome is to correct hypoxemia and treat the primary liver disease. Reports indicate that oxygen therapy and hyperbaric oxygen chambers may be effective for early-stage patients, helping to correct hypoxemia. For intrapulmonary anatomical shunts, such as arteriovenous fistulas, interventional methods can be used for treatment, such as placing coils, gelatin sponges, and other emboli to block the shunts, improve oxygenation, correct hypoxemia, and avoid surgery. Liver transplantation is an effective method for treating HPS; in terms of medication, amitriptyline dimethyl sulfate can be tried, at a dose of 50 to 100 mg each time, twice or thrice daily, for a continuous period of 3 to 5 weeks, to improve the ventilation/perfusion ratio of the lungs, thereby increasing blood oxygen saturation. In addition, drugs such as meclofenamate, octreotide, and garlic can also be tried, but their efficacy needs further verification.
